Request a care consultation

A thoughtful first conversation about the right support

Tell us what you are looking for and how best to contact you. This is an initial enquiry—not a clinical assessment—and you only need to share enough for us to understand how we may be able to help.

What happens next

Start with what matters

Once we receive your request, a member of the JJ Domiciliary team will review it and contact you using your preferred method. We can then talk through the person’s routines, interests, desired outcomes and the practical details of support in a more private setting.

You may enquire for yourself, for someone close to you, or in a professional capacity. If you are contacting us for another adult, please avoid including their name or private information in this initial form.

Submitting this form does not create a care agreement or guarantee availability. Any service would be discussed and agreed with the appropriate person before support begins.

This is not an emergency service

If someone is in immediate danger or needs urgent medical help, call 999. For urgent medical advice when it is not an emergency, use NHS 111. Do not rely on this form for an urgent response.

Your consultation request

Help us prepare for the conversation

The form is divided into four short steps. Fields marked with an asterisk are required.

Please do not include diagnoses, medication details, NHS or National Insurance numbers, financial information, identity documents, full dates of birth, detailed safeguarding information, or the full name of another person. We will request only what is genuinely needed later and through an appropriate private process.
